2. Information we collect

Contact information you give us when booking a ride: your name, phone number, email address, pickup and destination addresses.
Trip-related information: mobility level, chair type, oxygen needs, insurance identifiers when applicable, and any accessibility notes you share so drivers can serve you safely.
Payment information, which we process through a third-party payment processor (Stripe). We do not store full card numbers on our systems.
Website usage data: page views, referring site, approximate location (city-level), device type, and browser. We collect this only in aggregate.

3. How we use it

To schedule, dispatch, and complete the transportation you request.
To communicate with you about your ride, including confirmations, ETAs, delays, and receipts.
To bill you, your insurer, Illinois Medicaid, or the VA for services rendered.
To comply with legal, regulatory, and insurance record-keeping obligations.
To improve our website and dispatch operations.

4. Who we share it with

We share the minimum information necessary with: our drivers and dispatchers; insurers and payors you have authorized us to bill; medical facilities and case managers coordinating your care; our payment processor; and government agencies when required by law. We do not sell your personal information to anyone, ever.

5. HIPAA

Where we function as a business associate of a covered health-care entity, we handle Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Business Associate Agreements.

6. Cookies and analytics

Our site uses a small number of essential cookies to keep the site functional and, if you consent, a lightweight analytics cookie that counts anonymous page views. Details are in our Cookie Policy. You can withdraw consent at any time by clearing your browser storage or emailing us.

7. Your rights

You may request access to, correction of, or deletion of your personal information by writing to contact@sylhetambulanceservice.com. We respond within thirty days. Residents of certain states (including California under the CCPA/CPRA and Illinois residents whose data is subject to state law) have additional rights, including the right to know, delete, and opt out of the sale of personal information — which we do not do in any case.

8. Data retention

We keep trip records for seven years to satisfy insurance and tax obligations. Website analytics data is retained for fourteen months. Marketing contact information is retained until you unsubscribe.

9. Security

We protect your information with encrypted transmission (HTTPS), password-protected dispatch software, physical locks on paper files, and annual staff training. No system is perfectly secure, but we take this seriously.

10. Children

Our website is not directed to children under thirteen and we do not knowingly collect their information. Pediatric transport is booked by a parent or legal guardian.

11. Changes to this policy

We may update this policy from time to time. The "Last updated" date at the top reflects the most recent change. Material changes will be posted prominently on this page.
