Keystone LifeLine Medical Transport, LLC ("Keystone LifeLine," "we," "us," or "our") respects your privacy and is committed to protecting the personal and health information of our patients, families, and website visitors. • Date of birth, mobility status, and basic medical information needed to plan safe transport
• Insurance, Medicare, or Medicaid identifiers necessary to bill your trip
• Names and contact details of family members or caregivers you authorize us to speak with

• Pages visited and approximate geographic region (city/state level)

• To schedule, dispatch, and complete your transport safely
• To bill insurance providers or process direct payment

3. HIPAA and protected health information

Keystone LifeLine is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We maintain administrative, physical, and technical safeguards designed to protect your Protected Health Information (PHI). 4. Sharing information

We do not sell your personal information. We share information only:

• With healthcare facilities, physicians, or insurers involved in your care or billing
• With service providers (e.g. dispatch software, payment processors) under written confidentiality agreements
• When required by law, subpoena, or to protect the safety of patients or staff

5. Data retention

Trip records and billing data are retained for at least seven (7) years to comply with Pennsylvania Department of Health and federal requirements. Website analytics data is retained for up to 26 months.

6. Your rights

Depending on your state of residence, you may have the right to access, correct, or request deletion of personal information we hold about you, to opt out of certain marketing communications, and to receive a copy of records relating to your care. 7. Children's privacy

Our website is not directed to children under 13, and we do not knowingly collect personal information from them. Pediatric transport is arranged exclusively through parents or legal guardians.

8. Security

We use encrypted dispatch tablets, secured paper-record storage, role-based access controls, and annual staff HIPAA training. No system is perfectly secure, but we work to reduce risk.

9. Changes to this policy

We may update this Privacy Policy from time to time. The "Last updated" date at the top reflects the most recent revision. Material changes will be announced on this page.
